In the budget of 2018-2019˘, Government of India has announced the new health insurance scheme, called Ayushman Bharat Yojana – National Health Protection Scheme (AB-NHPS). The AB-NHPS envisions providing about Rs. 5 lakh of health insurance cover every Indian for healthier and fitter India. All secondary and tertiary care procedures will be taken care of under this scheme. The cost of the treatment will be covered under this scheme at both private and government hospitals. Moreover, all pre-existing diseases are expected to be covered, along with the costs of three days of pre-hospitalisation, as well as 15 days of post-hospitalisation.

The Government will pay this scheme’s premiums from the budget, which is estimated to be around Rs. 1,200 per family every year. However, the insurance companies have recommended the government to increase the premium up to Rs 2,500 annually.

The health minister will head this scheme that will work hand-in-hand with state governments. This new health insurance scheme is going to replace the existing Rashtriya Swasthya Bima Yojana (RSBY) that offers around Rs 30,000 health cover per year, and additional Rs 30,000 cover under Senior Citizen Health Insurance Scheme (SCHIS).

Under Ayushman Bharat Scheme, the government is going to allocate Rs. 10,000 to 12,000 crore annually to cover over 10.74 crore families. The beneficiaries of the scheme will be identified based on the Socio-Economic Caste Census (SECC) for covering up to 40% of India’s vulnerable population.

Ayushman Bharat is going to an entitlement-based policy that will be based on the deprivation criteria in the SECC database. In rural areas, eligible families will be those living in one room with kulcaroof and kucha walls; without any adult member between age 16 to 59; female-headed households; disabled family member; SC/ST households; and households that depend on their income from casual labour.  Even families without shelter, families living on alms, primitive tribal groups, manual scavenger families will also be included for the health cover. For urban areas, there are 11 defined occupational categories that are entitled under this scheme.

For controlling the cost of the scheme, the payments for treatment will be done on the basis of package rates that include costs associated with the treatment. There will be complete cashless, and paper-less transactions for the beneficiaries. The States and UTs will have the flexibility in modifying package rates within limited bandwidth for the requirement of the beneficiaries. Co-operative federalism is one of the core principles of the AB-NHPM scheme for giving flexibility to the State government to work through co-alliance.
